The transition from teenager to adult for students with developmental disabilities is a journey best taken with others. Through a coordinated set of activities and support from a team, students are able to reach their post-school goals.

Transition Planning is designed to identify a student’s abilities and interests and offer opportunities to develop those skills by exploring the world of work. The transition team makes it possible for students to learn what jobs are available and to help them decide where they want to work. This is done by providing experiences where you can actually do the job to see if it’s the right fit.

The end result is more independence and a self-determined life.
